Annual Meetings
Three registration options are:
• Online Registration (Sorry, but the registration system does not support MAC) Online registration is once again available. We apologize for the earlier technical difficulties that some of you experienced.
• Email, Fax or Mail the downloadable Registration Form
• Call the Registrars at 1-800-323-2688. Registration is available 8 a.m. – 5 p.m. Monday – Friday Central Time. When registering by phone, please decide which activities/sessions you are signing up for before you call and have your guest names and credit card ready. Refer to the Registration Form.
(as prompted on the left side of the login screen) which includes creating a unique username and password. (This is not your AAHM username and password.)
The person type field has the typical medical profession titles, e.g. Physician, Scientist, Mid-level Provider etc. Select “other” if your professional title/degree is not represented in the drop down list.
• Once you login, begin conference registration:
•Select your choice of activities
•Enter the names of any quests accompanying you
•Enter payment information ~ Accepted forms of payment: Discover Card, MasterCard and Visa. If you need to pay by check, please register by mail and include the check with the registration form.
• When you complete your registration and submit payment you should receive two emails;
one confirming your credit card transaction and one confirming your course registration. If you do not receive both of the email confirmations please contact the Mayo School of Continuous Professional Development to confirm your registration at 1-800-323-2688.
•Student Lunch, Saturday, May 1, Noon – 1:00 p.m.
During online registration, please indicate whether or not you plan to attend the Student Lunch. This year’s student lunch is at Victoria’s Ristorante, located near the Kahler Grand Hotel. You will be ordering off the menu and paying for your lunch directly to the restaurant that day.
